Alumni Awards are to be given based on service to the university, professional accomplishment or community service.  The list of potential award recipients comes from nominations by classmates, peers, faculty, and staff, with the most significant source being classmates.  Please help identify the most deserving alumni by nominating classmates who are worthy of an award.  Your assistance is a vital part of the alumni award process.

You may submit your nomination using the online form found below.  You may also email us your nomination.  If you prefer to mail or fax us your nomination, please print a copy of this version of the form.  

You may nominate more than one person.  Please submit a separate nomination for each individual.

The deadline for nominations is May 1,  2008.

Qualifications for the Young Alumni Award

  • Recent graduate from any UIU location who has demonstrated
    outstanding accomplishment in one of the above areas
  • Alumni up to 32 years of age are eligible
